It's all down to Russell. The Starz people have said it repeatedly -- they were willing to renew it back after Miracle Day, but that's when Russell's partner got sick, and everything was put on hold. He had other US-based projects that he put aside for his partner too, and they moved back to the UK so that they could get medical treatment for him through the NHS instead of paying through the nose as insured people in the States.
Russell holds all the cards, and that includes Ianto being permanently dead. (And in the light of the way the Ianto fans behaved after Children of Earth, it seems like RTD'll go out of his way to keep him that way just to spite them.)
I'm sure they'd all love to do the show again, and there are people who want to produce it, but RTD's pretty much said 'no' to anything/everything since his partner got sick. :(
